How do I get my Humana insurance card?

How do I get my Premium Saver card?

How do I file a claim with Premium Saver?

What is the relationship between Premium Saver and AmFirst?
Answer: Premium Saver is a supplemental insurance plan that is offered by Morgan White Group. Morgan White Group is a broker services company, and Premium Saver is one of their products. Sample Supports pays your invoices to Morgan White Group who is administering the Premium Saver product for you. And AmFirst is the insurance carrier who pays the claims for the Premium Saver product.  In short, AmFirst is the insurance carrier for the Premium Saver plan.

My family is experiencing a "qualifying event" and I need to change my insurance policy.
Answer:  Please submit the Benefit Enrollment - Qualifying Event form. If your qualifying event is due to a new birth or new adoption, your new benefits election/changes coverage will retroactively apply from the date of the birth/adoption. For all other qualifying events, your new coverage will start on the 1st of the following month. Special enrollments do not have the 60-day waiting period new hires do. 

I'm having an issue with my healthcare/prescription coverage. What can I do?
Answer:  Please contact our Healthcare Consumer Advocate: Baxter.Mouat@gbsbenefits.com. Baxter will help you in your dealings with Humana, Premium Saver and Mutual of Omaha. 

I'm having a hard time these days. I'm having trouble with my relationships. I need help with mental health. What can I do?
Answer: You can contact our Employee Assistance Program. The EAP is offered to all of our employees! 

How do I file a Short Term Disability Income Claim?
Answer: Here is the instructions from Mutual of Omaha. In addition, please let your supervisor or manager know about your potential disability right away and work through the Accommodation Process. 

When does my insurance take effect after I sign up?
Answer: New hire employees who qualify for our group insurance coverage has 60 days from the date of hire to sign up in Paylocity. Insurance coverage will begin on the 1st of the month following the 60 days. 
For employees with a qualifying event such as new birth/new adoption, insurance coverage will begin from the date of the qualifying event, retroactively. For qualifying event such as loss of coverage/divorce/marriage, insurance coverage will begin on the 1st of the month following the event. Qualifying event will also have 60 days of selection period, but not the 60 day wait period.

How do I make a claim in order to access my FSA/DCA funds if my cards don't work?

I want to enroll in 401K, how do I do that?
Answer: Sample Supports 401K plan is open to all employees who have worked 6 month and 500 hours. Enrollment begins on the 1st of the month after your 6 month and 500 hours of work. You can find out more information and instructions to enroll from this enrollment kit.

I have a 401K issue, who can I contact?
Answer: Please call TransAmerica at 800-755-5801, Monday to Friday 8am to 9 pm ET.

How do I request FMLA, sick leave?
Answer: Please let your direct manager know right away, and fill out the FMLA Request Form or the HFWA Request Form. Your Regional Director will follow up and contact you. 

I have Jury Duty, what do I need to do?
Answer: Please let your manager know, fill out the jury duty form and send your jury summon documents to employment@crystaljoys.com

Form 5500 Summary Annual Report: This annual report has been filed with the Employee Benefits Security Administration (ERISA) as required. The IRS receives form 5500. We are very proud to offer some of the best benefits package in the industry. Please refer to the Compliance Notice Packet for 2022

What this means is, our Health and Welfare plans such as medical, dental, vision, life insurance and voluntary disability benefit plans meet government standards and the premiums for each carrier must be shared, as these premiums also include profit and commission for the carriers and the brokers.
Premiums we paid for Humana, Principal, Aflac, American united Life Insurance Company are included in the 2021 Summary Annual Report.
